Which form do I need?

Form ST-119.1: The Exempt Purchase Certificate. New York sends it to your organization with its exempt organization certificate (Form ST-119). Make a copy, fill in our name and your purchase, sign it, and upload it here.
Government purchase: Government offices do not need a certificate: a purchase your office makes and pays for itself, on its own purchase order, is exempt. If you have a letter you usually give vendors, you can upload it.
Form ST-120: The Resale Certificate, for goods you resell. Get Form ST-120 from New York State (opens in a new tab).
